Your Health Information, Your Choice | Velatura
Patient Consent & Privacy

Your health information. Your decision.

Velatura connects the healthcare providers who care for you so your records can move with you. This page explains how that sharing works, and how to change your preference at any time.

01

About Velatura

Velatura is a secure health information exchange that allows participating healthcare providers, hospitals, and pharmacies to share your medical records so the people treating you have a fuller picture of your care.

We use encryption and access controls to protect your information, and we comply with HIPAA and applicable Missouri privacy law. Only staff at participating organizations whose jobs require it can view your records through the exchange, and every access is logged.

02

Your rights as a patient

If your provider participates in Velatura, your records are shared through the network by default so your care team can coordinate more effectively. You can change that at any time.

Default

Sharing is on unless you say otherwise

When a provider you see participates in Velatura, your health information is shared through the exchange to support coordinated, efficient care. No action is required from you to remain part of the exchange.

Your choice

You can opt out at any time

You may choose to stop sharing your health information through Velatura whenever you like, for any reason. Opting out does not affect the care you receive.

03

How to opt out

Opting out stops your health information from being shared through Velatura going forward. Requests are submitted through your provider.

Through your provider
  1. Ask your participating provider's office for the Velatura opt-out form, or bring a printed copy with you.
  2. Complete the form and your provider's staff will verify your identity in person.
  3. Your provider updates your consent status directly — either electronically through their ADT feed or by logging into the Velatura platform.

Changing your mind

If you've opted out and later decide you'd like your information shared through Velatura again, you can opt back in through a participating provider, who will update your consent status directly — either electronically through their ADT feed or by logging into the Velatura platform.

Opting back in does not restore records from the period you were opted out; it resumes sharing going forward.

Common questions

Does opting out affect the care I receive?

No. Your providers will continue to treat you and can still communicate about your care through other means, such as phone, fax, or their own medical record systems. Opting out only affects sharing through Velatura specifically.

Who can see my information through Velatura?

Only staff at participating healthcare organizations whose roles require it for treatment, care coordination, or other permitted purposes. Every access is logged, and misuse is subject to serious penalties.

Does one opt-out request cover my whole family?

No. A separate form is required for each individual who wants to opt out, including minors, who require a parent or legal guardian's signature.

Where can I learn more about how my information is protected?

Ask your healthcare provider for their Notice of Privacy Practices, which describes additional privacy options and protections beyond the HIE.
